As of season 2021/2022 minors (in principle) do not have to undergo a medical exam anymore under the condition that the child and parent/guardian can answer NO to all questions on the questionnaire. You do not have to provide the club or the federation with the filled out questionnaire, you only have to declare on the registration form that you can answer all questions with NO. The responsibility for completing the registration form correctly rests with the parent or guardian who signs the form.
Seniors still have to undergo a medical exam which is valid for 3 years (unless you had medical issues or you weren't licensed in one of the seasons).
